What Can I Expect from the Collaborative Care Management Program?
If your provider thinks that extra support for your mental health could help you, they might refer you to our program. Here’s what you can expect:
1. Referral and Initial Contact
After your provider refers you, one of our care managers will contact you via phone or video visit to discuss your needs, answer any questions and explain how the program works. This is also a chance to share how you’re feeling and what you hope to achieve—all from the comfort of your home.
2. Personalized Care Plan
Together with your primary care provider and our care management team, we'll create a personalized plan for you. This plan could include setting goals, helping you manage your medications and offering practical support to improve your mental health.
3. Convenient, Virtual Check-Ins and Support
You’ll have regular check-ins via phone or televideo visit with our care managers to talk about your progress, adjust your care plan if needed and make sure you’re on the right track. You can discuss any concerns or changes you’re experiencing, and your care manager will work with your primary care provider to make sure you’re getting the right support—all from your phone or computer, wherever you are.
4. Expert Guidance from Our Team
Our team includes a dedicated behavioral health care manager, chronic care managers and experienced psychiatrists who are available to provide specialized care when needed. Our psychiatrists can help with medication adjustments and provide recommendations to your care manager and primary care provider based on your progress. You’ll benefit from their expertise without needing to schedule a separate visit.
5. Completion and Future Care
Most patients complete the program in about six months, but every person's journey is different. When you finish the program, you and your provider will review your progress and decide on any next steps to keep you feeling your best.
