What Is Included in a Concierge Medicine Membership?
Extended Appointments
In traditional primary care settings, appointments are often 15 minutes or less. At FirstHealth Concierge, you will complete an extensive health questionnaire before your first appointment that delves into your medical history, including any medications you take as well as social history and social determinants.
Your provider reviews your information, so your first appointment is a detailed discussion on your health care concerns and needs. Together, you develop a map for your future health care plan.
Our providers focus on the underlying causes of health problems rather than the symptoms, allowing them to develop a multifaceted, customized plan that yields better long-term outcomes.
Preventive Care and Annual Exams
Our providers complete your annual wellness exam as well as manage any chronic illnesses such as diabetes or hypertension, develop a preventive plan and offer same-day availability for sick visits.
Direct Physician Access
Our office is open Monday through Thursday from 8 a.m. – 5 p.m. and Friday from 8 a.m. to noon for routine appointments. Accommodation for same-day appointments for any urgent needs will be met to the best of our ability.
Any appointments can be scheduled by contacting the office or sending a message through MyChart and our staff will address it promptly.
An on-call physician is also available after hours for telephone consultations. Because we are a FirstHealth clinic, our physicians collaborate with the inpatient hospitalist team to help coordinate your care in times of hospitalization.
Care Coordination
FirstHealth Concierge offers a patient advocate dedicated to coordinating your care with other specialists, test scheduling and any other needs you may have.
