Target Audience: All physicians and other interested health care professionals

Speakers: Amy Graham Vice President, FirstHealth Strategy and Innovation, Chris Miller Administrative Director, FirstHealth Community Health Department and Roxanne Elliott Policy Director, FirstHealth Community Health Department Objectives.

At the completion of this activity, the participant should be able to:

• Define population health

• Describe what population health looks like in our region, to include public health, the Community Health Needs Assessment data compared with hospital data, HealthNC + data and quality data

• Use the evidence based practices that are being implemented by FirstHealth to improve population health Professional Practice Gap to be filled: Lack of information on initiatives and strategies to improve the health of FirstHealth’s communities
