Cardiac CARE-Net
The Cardiac CARE-Net provides stipends for medications and transportation for patients who have been identified through cardiac rehab or their physician office. The goal is supporting the cost of these items to ensure that the patient remains compliant in follow-up care and is taking the proper medications to avoid an unnecessary hospital readmission.
