You can most improve on the provider knowing the patient's history before they make an appointment. My provider had no documentation of anything, so she wasn't updated on any of my current or past history. And it was most disconcerting because it's hard to talk about anything or make an appropriate judgment on what to do. So I found the lack of her knowledge on my history was a little disconcerting. That's all.
