The FirstHealth Diabetes Self-Management Program offers one-on-one counseling and group classes to help patients understand their disease. Patients benefit from time with both a nurse and dietitian as well as group classes and follow-up sessions when needed.

One month, three month or six month coaching by a Certified Diabetes Educator includes weekly coaching, education and support via one on one office visits, phone and email consultations. Fees may apply.
The FirstHealth Diabetes Self-Management Program is an American Diabetes Association-accredited educational series offering services in Moore, Richmond, Montgomery and Hoke counties.
