Last fall, the NET-AIM team began leading nursing on our Magnet Journey. One of the top priorities identified by the group was the need to define our nursing Model of Care.

After studying “Relationship Based Care”, the team felt that practice roles outlined in this work best described the care we presently provide to our patients and families.

A new model, “Our Nursing Staff CARES”, was developed and endorsed by the group.

The CARES model incorporates both the concepts we studied and our core purpose, “To Care for People”. The visual image used to depict the model places the patient at the center of interlocking circles of excellence.

Principles of care defined in the new model of care include:

 C  Collaboration
The patient’s primary nurse communicates with colleagues and other disciplines to insure coordination of care and mobilization of appropriate resources.

 A  Advocacy
The patient’s primary nurse assesses needs and wishes of the patient and family to insure they receive the physical, emotional and spiritual support to help reach their desired outcome.

 R  Research
The patient’s primary nurse incorporates the use of evidence (nursing research) in daily practice, evaluates results and initiates changes that will improve the quality of care for patients and families.

 E  Education
The patient’s primary nurse provides information, assists patients and families with understanding disease processes and current plan of care, and supports consultation with health team partners to insure patients have the resources to make informed decisions.

The patient’s primary nurse facilitates teaching the patient and family how to safely care for themselves, during their acute care stay and following discharge.

 S  Safety
The patient’s primary nurse continuously collaborates with colleagues to assess, monitor and intervene for the patient to prevent complications, promote healing and optimize safe outcomes.
