Please arrive a few minutes before your scheduled appointment and check in with the front office receptionist. We also ask that you bring the following items to your office visit:
- All medications you are currently taking
- Current insurance information, including your insurance card
- If you have more than one insurance carrier, please let us know which plan is primary.
If you must cancel an appointment, please call at least 24 hours ahead of your scheduled appointment. This will allow other patients to be scheduled for an appointment in your place.
Payment is expected at the time of service. You may pay by cash, personal check, Visa or MasterCard. Payment plans are available; see the patient accounts representative prior to your appointment for more information.
FirstHealth Family Care will file your primary and secondary insurance. However, all deductibles and co-payments should be paid in full at the time of service.
FirstHealth Family Care providers accept assignment on all Medicare charges. Patients are responsible for their annual deductible and 20 percent of the allowable charges at the time services are rendered.
