FirstHealth Home Health offers home health services in the comfort and security of the patient’s home, allowing patients to reduce the amount of time spent in the hospital while still being treated by highly skilled, specialized health professionals.

Services are available throughout Moore, Montgomery, Richmond, Lee, Anson and Scotland counties

 
 

Services provided through Home Health include:

  • Skilled Nursing including disease/medication management and education; wound/ostomy care; IV infusion therapy; and pediatric care
  • Physical Therapy/Occupational Therapy including orthopaedic/neurological, post-hospitalization and pediatric rehabilitation; home safety/falls prevention; and life management skills assessment and training
  • Speech Therapy for post stroke care, speech/swallowing training and cognitive retraining
  • Medical Social Worker Services for long-term planning, referral to community resources and assistance with financial concerns
  • Home Health Aides for assistance with personal care
  • Nutritional Consultant for disease-specific nutritional assessment and education

Telehealth Monitoring
FirstHealth Home Health provides telehealth monitoring for select high-risk patients with congestive heart failure or wound care needs that need constant monitoring. This state-of-the-art technology can send data over a simple phone line to a central monitoring station where a registered nurse can review it. The date includes blood pressure, temperature, blood sugar, pulse, weight and blood oxygen levels. Based on that data, the nurse can determine the best possible course of action.
