A community-based program for disease management, medication assistance, education and referral services

The Community Case Management Program is a new program open to eligible patients with any chronic disease or health care need who would benefit from medication assistance, education, referral to community resources, and general guidance in management of their chronic disease or health care need.

The program serves patients living within a 30-mile radius of FirstHealth Moore Regional Hospital, including areas in Moore, Montgomery, Richmond, Scotland, Hoke and Lee counties.

A registered nurse coordinator and highly qualified, trained volunteers staff the Community Case Management Program.

If you have a chronic disease process or health care need and would like to be assessed for admission to the Community Case Management Program, please call the Program Coordinator at (910) 715-1275. Physicians, nurses, case managers, ancillary staff, friends or family members can also refer patients to the Community Case Management Program.

Once a referral is received and the patient is determined to be eligible for enrollment into the Community Case Management Program, the Program Coordinator will assign a volunteer who will conduct scheduled, scripted follow-up calls. Through this process, the volunteer will aid the Program Coordinator in recognizing and coordinating continued patient assistance needs. The program is free, and eligibility is based on established admission criteria.

Specific services include:

  • Scheduled follow-up phone calls to assess disease management and, when needed, referral for medical intervention
  • Home assessment as necessary
  • Medication management and assistance with procurement of medications for indigent or underinsured patients
  • Referral to appropriate community resources
  • Health-related education
  • Assistance with obtaining/arranging transportation to medical appointments
  • Telehealth services for at-risk patients

The Community Case Management Program is a part of the Foundation of FirstHealth Hospitality Services Program.
