Health Care for the Whole Family

Family medicine is the medical practice that provides health care regardless of age or sex while placing emphasis on the family unit. Think of the FirstHealth Family Care Centers as your family’s doctor’s office, treating all ages—from babies to grandparents.

Appointments

The FirstHealth Family Care Centers are happy to accept new patients and always appreciate your referrals of friends and family.

To better serve you, the primary care providers at the Family Care Centers prefer to see patients by scheduled appointment. Please check with each office location for office hours and to schedule an appointment ahead of time. Patients without an appointment may be worked in and will be seen as soon as possible.

Please arrive a few minutes before your scheduled appointment and check in with the front office receptionist. We also ask that you bring the following items to your office visit:

• All medications you are currently taking
• Current insurance information, including your insurance card

If you have more than one insurance carrier, please let us know which plan is primary.

Appointment Cancellation

If you must cancel an appointment, please call at least 24 hours ahead of your scheduled appointment. This will allow other patients to be scheduled for an appointment in your place at the Family Care Center.

Payment is expected at the time of service. You may pay by cash, personal check, Visa or Mastercard. Payment plans are available. Please see the patient accounts representative prior to your appointment for information.

Insurance

As a courtesy to you, the Family Care Center will file your primary and secondary insurance. However, all deductibles and co-payments should be paid in full at the time of service. Please bring your valid proof of insurance card each time you visit the Family Care Center.

The physicians and providers at the Family Care Centers accept assignment on all Medicare charges. Patients are responsible for their annual deductible and 20 percent of the allowable charges at the time services are rendered.

The Family Care Centers currently participate in several managed care plans. Please call or contact your carrier to confirm that your Family Care Center participates with your plan.
